Outpatient, IOP & PHP Explained

Outpatient treatment allows you to receive care while living at home. There are several levels of intensity, from standard outpatient to partial hospitalization. Understanding these options can help you find the right fit.

Standard Outpatient

The least intensive level of care, standard outpatient typically involves:

  • Hours per week: 1-9 hours
  • Schedule: 1-2 sessions per week
  • Focus: Individual therapy, group therapy, or both
  • Best for: Those with stable living situations, strong support systems, and lower acuity needs
  • Flexibility: Can often work around job or school schedules

Intensive Outpatient Program (IOP)

A step up in intensity, IOP provides more structure while still allowing you to live at home:

  • Hours per week: 9-20 hours typically
  • Schedule: Usually 3-5 days per week, several hours per day
  • Focus: Group therapy, individual sessions, skill-building, education
  • Best for: Those stepping down from higher levels of care, or those needing more support than standard outpatient
  • Timing: Many programs offer morning or evening sessions

Partial Hospitalization Program (PHP)

The most intensive outpatient option, sometimes called "day treatment":

  • Hours per week: 20+ hours, often 5-7 hours daily
  • Schedule: Usually 5 days per week
  • Focus: Intensive therapeutic programming similar to residential, but you return home at night
  • Best for: Those needing intensive support but with stable, safe living situations
  • Medical care: May include medication management and psychiatric services

Choosing the Right Level

Consider these factors when selecting an outpatient level of care:

  • Stability: Is your home environment supportive of recovery?
  • History: What levels of care have worked (or not worked) in the past?
  • Obligations: What work, family, or school commitments do you have?
  • Support: Do you have people who can support your recovery at home?
  • Clinical recommendation: What does your treatment team suggest?

Stepping Down Through Levels

Many people move through levels of care as they progress:

  1. Residential or PHP for initial intensive treatment
  2. IOP for continued structure with more independence
  3. Standard outpatient for ongoing support and maintenance
